HOW TO USE
Download the file and open it in a PDF reader that supports AcroForm fields. Save a working copy, type your changes, save and reopen to confirm. Some browser previews limit editing and saving. No paid design subscription or design account is required.
Print at Actual size / 100% on the paper size named in the file, then cut along the card edges. Plain paper works; card stock or lamination is optional. Paper and physical supplies are not included.
